11/21/2023 EnchantaVaughn Approved QMHPA arrived to the home go Ms. Cosby to conduct an individual session with Ms. Cosby. Upon arrival at the home, Ms. Cosby presented with positive affect evident by her facial expression and physical demeanor. QMHPA informed Ms. Cosby that this session was going to continue to address her individualized goals. Ms. Cosby did not present any symptoms of SI or HI at the time of the session being conducted. QMHPA utilized active listening to provide Ms. Cosby with constructive feedback regarding her mood. QMHPA assisted Ms. Cosby with learning how to express her emotions appropriately. QMHPA educated Ms. Cosby on the importance of developing healthy interpersonal relationships. QMHPA emphasized the importance of Ms. Cosby using assertive communication styles when meeting new people; and to avoid aggressive styles. QMHPA supported Ms. Cosby in understanding how engaging in more social settings can increase her daily living and reduce isolation. Towards the end of the session, counselor encouraged Ms. Cosby not to isolate herself from others. Cosby learned how to express her emotions in an assertive manner and not be aggressive towards others. Ms. Cosby required some assistance with processing the importance of healthy interpersonal relationships. Ms. Cosby noted that she did not like people and like being by herself. Ms. Cosby was encouraged to seek age appropriate social settings that could increase her support system. Ms. Cosby was able to recall how to express her thoughts and feelings assertively. Ms. Cosby practiced utilizing assertive skills such as looking one in the eye, being mindful of ones’ tone and posture. Ms. Cosby was encouraged to practice these skills independently. Ms. Cosby notes her struggles with controlling her emotions when communicating with her family and peers. Ms. Cosby is reminded to work on building a healthy relationship with her family and to avoid conflict in the home.
